Participant Waiver and Release Form Terms and Conditions * As the undersigned participant in any exercise program implemented by Dean Seda, a nationally certified personal trainer, as a condition of my participation, hereby agree to waive any and all legal claims for personal injury, against Dean Seda, that may arise from my participation in these exercise programs. I understand that I am waiving and releasing Dean Seda from any and all claims, judgments, liabilities and expenses, including but not limited to legal and court costs. I have consulted with my physician as to my ability to participate in an exercise program implemented by Dean Seda in different forms of fitness activity I may engage in and have secured his/her approval. I agree to limit my participation in the exercise program implemented by Dean Seda to a level of activity within my abilities.
